Report a Fraud Loss

Please complete the Fraud Loss Form and send it to ICBA Bancard along with all the required material below, in order to report a fraud or loss:

required Please note: All informatiion below is required to submit a claim.

  1. Copy of Close-Out Package including:

    • Copies of all fraudulent sales drafts with legible signatures.

    • Copy of signed affidavits for each cardholder and/or authorized user.

  2. Copy of Customer Billing Statement with all fraudulent transactions highlighted.

  3. Copy of police report. (For stolen card losses )

  4. Adding machine tape showing all fraudulent transactions. (Total must equal amount claimed )

  5. Letter from cardholder(s) stating that the valid cards are still in his or her possession. (For counterfeit claims )

  6. An imprint/copy of the card(s). (For counterfeit claims )

  7. Written detailed statement: indicating the circumstances surrounding the loss or theft of the card.

Credit CARD Act: Simplified Q&A Format Summary for Customers on the Fed's Final Gift Card Rule

The Credit CARD Act that was signed into law last May included provisions applicable to gift cards, and the Federal Reserve Board issued the final rule concerning gift cards last month.  The U.S. Public Policy Team has prepared a simplified summary in a question and answer format for your information and for you to share with customers.
