Report a Fraud Loss
Please complete the Fraud Loss Form and send it to ICBA Bancard along with all the required material below, in order to report a fraud or loss:
Please note: All informatiion below is required to submit a claim.
- Copy of Close-Out Package including:
- Copies of all fraudulent sales drafts with legible signatures.
- Copy of signed affidavits for each cardholder and/or authorized user.
- Copy of Customer Billing Statement with all fraudulent transactions highlighted.
- Copy of police report. (For stolen card losses )
- Adding machine tape showing all fraudulent transactions. (Total must equal amount claimed )
- Letter from cardholder(s) stating that the valid cards are still in his or her possession. (For counterfeit claims )
- An imprint/copy of the card(s). (For counterfeit claims )
- Written detailed statement: indicating the circumstances surrounding the loss or theft of the card.
